Where can I find vendors for pull-out pantries as described on p79 of "Not So Big Solutions"?

... and does anyone extend this concept to bookshelves instead of pantries?

For this kind of thing, you will need to get a local craftsperson to custom design the pull-out pantry or bookshelves for you. It sounds expensive, but often times custom companies can be more reasonable than buying something pre-made, as you will be cutting out the middle-man.
